  • Building Community-Owned Systems

Community-owned systems. Designed to endure.

OFSI is the CSR arm of Omega Healthcare, working across health and livelihoods. Together with grassroots organisations, we build systems communities can own. Trained local navigators we call SARTHIs help communities access, navigate, and strengthen these systems.

Why OFSI exist?

Health and livelihood systems are more likely to endure when communities lead them. OFSI combines its corporate philanthropic investment with hands-on partnership to make this possible.

We work alongside grassroots organisations with established community relationships, co-create programs together, train local people to lead them, and support the work through funding, learning, and long-term partnership.

  • Our Model

Aligning CSR commitments with philanthropic work.

OFSI sits at the intersection of corporate social responsibility and philanthropy-led work. We bring the discipline and accountability of a corporate foundation, and the patience and community focus of long-term engagement.

This model lets us act with rigour, move with trust, and invest in what communities can actually carry forward.

Our Story
  • The SARTHI Difference

Change led from within.

SARTHIs are community members selected and trained to help people access healthcare, livelihood opportunities, and the services that support them.

As trusted local navigators, they connect people with the programs, resources, and support they will avail around their ecosystem.

Present across both AAROGYA, our health systems pillar, and UDAAN, our livelihood systems pillar, SARTHIs are a defining feature of OFSI's approach.

  • What We Do

Two areas. One integrated approach.

AAROGYA (health) and UDAAN (livelihoods) are OFSI's two thematic areas. Together, they create opportunities for leadership and agency through SANKALP.

AAROGYA

Health Systems

Community-centred health systems built around promotive and preventive health, primary healthcare, nutrition, mental wellbeing, inclusive care, and social protection. SARTHIs support awareness, follow-up, and continuity of care, helping families stay connected to the services they need.

UDAAN

Livelihood Systems

Community-led livelihood systems built around skills development, enterprise, market access, and employment pathways. SARTHIs help people navigate these pathways, connect with opportunities, and grow sustainable livelihoods.

  • LEADERSHIP THAT EMERGES

SANKALP: Where leadership takes shape

Leadership and agency grow through participation, shared responsibility, and community action across AAROGYA and UDAAN. SANKALP brings together OFSI's flagship leadership initiatives while strengthening the confidence, participation, and local leadership that emerge through this work.

She Matters

A residential leadership immersion for grassroots women. What began with 31 women in the first cohort has grown to 77 in 2025, with women from the She Matters ecosystem leading peer learning for more than 1,000 community members.

Sangam

Our annual convening of community leaders, grassroots practitioners, policymakers, and funders. Sangam 2025 brought together 200+ Omega employees and stakeholders around the theme Beyond Livelihoods: Seeding Futures, Shaping Journeys.

Agni Action Lab

Immerses young professionals in grassroots realities while exposing them to the role of CSR and philanthropy in enabling lasting social change.

  • How We Work

Partner. Pilot. Progress.

Success is measured by what communities can sustain over time.

Partner

Partner

We listen first. We engage communities and grassroots organisations to understand local realities, identify priorities together, and build trust before building programs.

Pilot

Pilot

We test small and learn fast. Promising ideas are tried, adapted, and refined through continuous feedback from the field.

Progress

Progress

We build for continuity. As pilots mature, we work with partners to build institutional capacity and embed approaches within local systems until communities can sustain the work themselves.

  • Voices from the ground

We joined Skill Nest as learners. Today, we train 300 women in Aari work together. What began as a skill course has become a leadership journey.
